Which way round should the diagram be?
Vertically, with the low E string on the left, is the standard for chord boxes. Horizontal diagrams are for scales and riffs along the neck.
Should open strings be marked?
Yes — an o above the nut for open, an x for muted. Leaving them blank is ambiguous, and a ringing muted string is the most common beginner error.
