How do I actually turn this into my auto-reply?
Copy the generated text and paste it into your email client's "Automatic replies" or "Vacation responder" settings — this tool writes the message, not the auto-reply rule itself, since that setup differs by provider (Gmail, Outlook, etc.).
Should I include exact dates in a sick-leave message?
Not necessarily — for unplanned sick leave, many people leave the return date vague ("out of the office due to illness") rather than committing to a specific date. Edit the generated text if you'd rather not include one.
Is the contact person field required?
