Is my health data stored?
No. Everything stays in this page and nothing is transmitted, which also means closing the tab loses it — export or print anything you need to keep.
What if I do not know exact dates?
Approximate is fine. "Spring 2021" placed on a timeline is far more useful to a clinician than a gap, and it can be corrected against records later.
Is this a medical record?
No. It is a personal summary for your own use in appointments. Your clinical record remains whatever your providers hold.
