The formula
Medical bill liability is calculated across three sequential tiers:
Remaining Deductible = Max(0, Deductible − Amount Paid YTD)
Deductible Share = Min(Bill, Remaining Deductible)
Coinsurance Share = (Bill − Deductible Share) × (Coinsurance % / 100)
Patient Payment = Min(Deductible Share + Coinsurance Share, Out-of-Pocket Max Remaining)

How do health insurance deductibles work?
A deductible is the set dollar amount you must pay out-of-pocket for covered medical services each calendar year before your health insurance plan starts paying benefits.
What is coinsurance versus a copay?
A copay is a fixed flat fee (e.g., $30 per doctor visit), while coinsurance is a percentage share (e.g., you pay 20%, insurance pays 80%) of the negotiated medical cost after your annual deductible has been met.
What happens when I reach my Out-of-Pocket Maximum (OOP Max)?
Once your total payments for deductibles, coinsurance, and copays reach your annual Out-of-Pocket Maximum, your insurance policy covers 100% of all eligible in-network medical costs for the remainder of the plan year.
Do monthly insurance premiums count toward my deductible or OOP max?
No. Monthly premium payments are the fee you pay to maintain your insurance policy and do not count toward your annual deductible or out-of-pocket maximum.
What if my deductible is already fully met for the year?
Enter your amount paid year-to-date as equal to or greater than your deductible, and the calculator applies coinsurance to the full bill from the start, since the deductible share is zero.
