How Asthma Control Check works

  1. Answer all five questions about the past 4 weeks — activity limitation, shortness of breath, night waking, rescue inhaler use, and your own rating of control.
  2. Each answer is worth 1 (all of the time / not controlled at all) to 5 (none of the time / completely controlled).
  3. The five scores are summed to a total out of 25, calculated instantly and locally as you answer.
  4. The total is grouped into a plain-language band so you can see at a glance whether it suggests well-controlled, not well-controlled, or very poorly controlled symptoms.

The scoring bands

A total of 20-25 suggests well-controlled asthma, 16-19 suggests asthma that is not well controlled, and 5-15 suggests very poorly controlled asthma — these thresholds mirror the bands used by standard 5-item control-test questionnaires. A single low-scoring question (for example, frequent rescue inhaler use) is worth flagging to a clinician even if the total lands in the "well controlled" range.

What scale is the score on?

The five questions are each scored 1-5, for a total between 5 and 25 — higher means better control, mirroring the scoring convention used by standard 5-item control-test questionnaires.

Is a low score a diagnosis?

No. This is a self-assessment for general awareness, modeled loosely on standard control-test questionnaires — it does not diagnose asthma severity or replace spirometry, a clinical exam, or a conversation with your doctor.

Why does rescue inhaler use count toward the score?

Frequent reliever/rescue medication use is one of the clearest day-to-day signals of poor control in standard control-test questionnaires, alongside night waking, activity limitation, and breathlessness — it is weighted the same as the other four questions here.
